Certification Process is as follows:
 

1. You will need to provide us with your background information for our records.
   You can download a Patient Intake Form here.
    
2. You will need to complete and provide our physicians with a medical history assessment.
   
    
3. If you have medical records or documentation supporting your medical condition you will need to provide our physicians with your medical records or documentation.
   
    
4. Our physicians will then review your medical history along with any medical records. Medical records must be current and address your qualifying condition.
    
5. Once your pre-screening is complete, you will meet with one of our physicians to further discuss and review your condition. Our physician will then decide if you qualify for the use of medical marihuana, and if you do, complete a Physician Certification form for you. Patients should bring a valid Michigan ID or Michigan Drivers License. Insurance companies do not cover medical marihuana certifications or consultations.
    
6. You will then need to apply for a Registry Identification Card from the Michigan Department of Community Health (MDHC) – Michigan Medical Marihuana Program (MMMP). You must then send your completed Application Form For Registry Identification Card, Physician Certification, and other required documentation to the MDHC. You can download an Application Form and get other information about the MMMP at www.michigan.gov/mmp.
    
7. A $100 registration fee (checks or money orders only) is also required with your Application, payable to the State of Michigan – MMMP. The registration fee can be reduced to $25 for patients enrolled in Medicaid or receiving SSI or SSD (copies of qualifying documentation must be included)
